D., FAAN, Associate Research Scientist A pilot demonstration to improve the quality of care for patients and families in non-hospice home care agencies through design, implementation, and evaluation of a model education program. This educational project will develop and evaluate a model education program which can be disseminated widely through home care agencies to improve care for terminally ill patients and families. Over two years, a curriculum will be developed and testing in 5 home care agencies representing various models of home care delivery and providing care to culturally diverse populations. | $76,650 | One Year |
